2026 Update Form

If you have not done so already, please download and complete our 2026 update form and bring it with you to the office. We also we will need to make a copy of your insurance card, even if it has not changed. Thank you for your cooperation.

New 2026 Cancellation Policy

You will be charged $80 if you cancel an appointment less than 24 hours ahead of time, or if you miss an appointment. Your insurance company/HSA account will not pay for these charges. Once you schedule your appointment with any of our providers, that time has been set aside for you alone and is not available to other patients who need to be seen. We understand that emergencies arise and we will review exceptions on a case-by-case basis.
